How to Become an IHSS Provider
A provider is someone who takes care of a recipient under the IHSS Program (In Home Supportive Services). See below all requirements to become an IHSS provider.

You must complete all steps from SOC 426 within 90 days to become a provider. See below for forms and other documents.
Call Community Service Solutions if you need a provider or for any other questions.

Complete & sign the IHSS Program Provider Enrollment Form (SOC 426) and return it to CSS before you start your orientation.
STEP 2

Criminal background check & get fingerprinted (bring Live Scan Form) *** Only Cash / Check

Attend Provider Orientation with Community Service Solutions. Both Mono County or Inyo County offices are available.

Bring to Provider Orientation:
-Form of ID (drivers license, passport, state ID, or tribal ID card)
-Social Security card or Birth certificate
STEP 4

After completing the Provider Orientation, sign an IHSS Program Provider Enrollment Agreement (SOC 846)
